File Uploader Pro 4.1

You set the file types that are allowed to be uploaded and File Uploader Pro will handle the rest!
You can upload from One to as Many as you need, 1-12 files at one time, along with as many extra fields of information that you might want to go along with them! The program then sends the user a confirmation email with the names of the files, sizes, date and time. along with the rest of the forms results.

It then emails you with the same information, but with links to the uploaded files so that you can view and or download them with the click of a link!
